Comments:

An unannounced monitoring inspection was conducted today approximately 12:05 pm-2:00 pm. There were 7 children, totaling 16 points, supervised directly by the provider. The physical plant, 3 caregiver/household member files, 3 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs at this time. The children were observed napping. Description:	Based on observation, and staff statements, a written record of attendance was not kept. Evidence: The provider was not to provide a written attendance for each day this week.
Plan of Correction:	I will begin taking attendance daily.
Standard #:	22VAC40-111-50-C
Description:	Based on observation, the provider failed to ensure that all children's records were kept confidential. Evidence: The inspector observed that the supplemental page that contains the children's name from the previous inspection (7/13/2018 and 7/09/2019) was posted on the parent board. Supplemental page states that it is not to be posted.
Plan of Correction:	Supplmental page was removed during the inspection. I will not post supplemental page
