An unannounced renewal inspection was conducted from 9:30 am - 12:30 pm with the provider and her assistant. There were nine children in care, ranging from nine months to four years of age, for a total of 22 points. The children were observed engaged in a variety of activities including coloring, dress up, free play, play doh, washing hands and eating lunch. Lunch served today: cheese/pepperoni pizza, peas, pineapple and milk. Three younger children were observed napping. Four children's records and two caregiver records were reviewed. The first aid kit was reviewed and in compliance. The emergency drill log was reviewed and in compliance. Two medications and authorization forms were reviewed and in compliance. The provider has current Medication Administration Training (MAT) certification as well as CPR and First Aid. Standard #: 22VAC40-111-120-B Description: Based on review of two caregiver records, it was determined that not all records contained all required information. Evidence: the record for caregiver #2 did not contain verification of age or 2 letters of recommendation. Plan of Correction: Will obtain the missing information.
Standard #: 22VAC40-111-60-A Description: Based on review of four children's records, it was determined that not all records contained all required information. Evidence: the record for child #3 (date of first attendance: 12/1/17) did not have the addresses for two emergency contacts. Plan of Correction: Will ask the parents for the missing information.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of two caregiver records, it was determined that not all caregivers had an original central registry report within 30 days of employment. Evidence: the record for caregiver #2 (date of hire: 11/26/19) did not have a central registry finding report. Plan of Correction: Will submit paperwork for a Central Registry check, and ensure caregiver does not return to work until the findings have been received.
Standard #: 63.2-1720.1-B-2 Description: Based on review of two provider/caregiver files, and an interview with the provider, it was determined that not all caregivers had an Office of Background Investigations (OBI) fingerprinting-based criminal history record check. Evidence: the record for the caregiver #2 did not have an OBI fingerprinting determination letter. Plan of Correction: Will schedule caregiver for fingerprinting and ensure caregiver does not return to work until the findings have been received.
