An unannounced monitoring inspection was conducted today at approximately 12:40pm. There were 11 children (30 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Per Provider there were no medications on site today. Children were resting. During inspection 1 child awoke and was comforted by caregiver. There was an adequate supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-60-B Description: Based on review of child records it was determined that all required information was not maintained.
Evidence: During inspection child A was missing the work phone numbers for 2 of 2 parents listed.Plan of Correction: They both work from home. I will have them indicate that on their record.
