Technical Assistance:
Licensing inspector had a discussion with the provider regarding the physical environment; suggested hanging posters/pictures and the children's artwork. Also suggested playing music for the children throughout the day and during naptime.

Provider was encouraged to attend the free Phase II training that is offered by DSS in the fairfax licensing office.

Comments:
An unannounced renewal inspection was conducted today between 11:05am and 2:15pm. There were 6 children present with the Provider for a total of 19 points; this was not within the supervision guidelines. The children were observed playing with blocks, singing songs, dancing, throwing/catching balls, eating lunch (brought from home) and napping. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-830-B
Description: Based on record review and interview, shelter-in-place procedures were not practiced a minimum of twice per year.
Evidence: Two shelter-in-place drills were not documented on the provider's emergency drill log. The provider stated she thinks that she did them but that she must've forgot to write them.

Plan of Correction: I'll do them and write it down.

Standard #: 22VAC40-111-870-G
Description: Based on observation, tables were not cleaned after each use.
Evidence: The table that the children were observed eating lunch at was not sanitized after an activity was complete and prior to when the children began eating.

Plan of Correction: I'll start doing it today before snack.

Standard #: 22VAC40-111-900-B
Description: Based on observation, children were allowed to drink while laying down.
Evidence: At the start of naptime, the provider was observed laying the children down on their sleep mats and handing 4 out of the 6 children bottles of milk to drink.

Plan of Correction: I will stop doing it tomorrow.

Standard #: 22VAC40-111-980-A
Description: Based on record review and interview, the provider has not obtained written permission to take the children off the premises of the family day home for regularly scheduled trips to the playground that is down the street from the family day home.
Evidence:
1. 6 out of 6 children's records did not contain written permission from the children's parents to leave the family day home.
2. The provider stated that she occasionally takes some of the children to the playground down the street to play.

Plan of Correction: I'll have the parents sign the form today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #1 file does not contain the child's first date of attendance.
2. Child #2 file does not contain an address or phone number for the families second emergency contact or the families medical insurance policy number.
3. Child #6 file does not contain documentation of an annual review. The most recent documentation of review is dated 2/4/2019.

Plan of Correction: I'll have all missing information done today.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider has not obtained documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child #1 (DOB: 11/29/2018) most recent documented immunizations are dated 6/13/2019.
2. Child #5 (DOB: 6/4/2018) most recent documented immunizations are dated 6/7/2019.

Plan of Correction: The parents are waiting for the doctor to send updated immunizations.

Standard #: 22VAC40-111-200-A
Description: Based on interview, the provider has not orientated their assistant by the end of their first week assuming job responsibilities.
Evidence: The provider stated that they were unaware that they needed to do orientation training with their assistant.

Plan of Correction: I'll complete this next week.

Standard #: 22VAC40-111-210-A
Description: Based on record review, caregivers have not obtained 16 hours of annual training in areas relevant to their job responsibilities.
Evidence:
1. The provider attended 12 hours of training during 2019.
2. The provider stated that their assistant did not attend any training during 2019.

Plan of Correction: We'll complete it this year.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas of the home are not maintained in a clean, safe and operable condition.
Evidence: There is a hole, approximately an inch in diameter, on the wall next to the door to the garage in the children's play area. The hole has broken through the sheetrock.

Plan of Correction: We'll finish this this weekend.

Standard #: 22VAC40-111-430-A
Description: Based on observation, children under two years age have access to stairs.
Evidence: 5 out of 6 children observed in the provider's care are under the age of two years. The provider has a tension gate leaning up against the wall. The children had access to the stairs.

Plan of Correction: We're planning on putting a door at the bottom of the stairs. I'll start putting using the gate.

Standard #: 22VAC40-111-570-A
Description: Based on observation and record review, the provider did not ensure that they did not exceed 16 points.
Evidence: During today's inspection the provider was observed caring for 19 points worth of children - Child #1, 3 points. Child #2, 3 points. Child #3, 4 points. Child #4, 3 points. Child #5, 2 points. Child #6, 4 points.

Plan of Correction: I have one child who is part-time, I'll give the family a two-week notice to find other care for the child.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water after diapering.
Evidence: Two children were observed having their diapers changed. The children's hands were not washed after their diaper changes were complete.

Plan of Correction: I'll start doing it.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface is not nonabsorbent, washable and sanitized after each use.
Evidence: Two children were observed having their diapers changed on a soft, fabric blanket. The diapering surface was absorbent and could not be washed or sanitized after each diaper change.

Plan of Correction: The mat is double-sided, the opposite side has a plastic surface. I'll start using the plastic side. I'll sanitize after each diaper change.
