Comments:
An unannounced monitoring inspection was conducted today from 9:10am to 11:10am with the provider. There were seven children in care, ranging from 20 months to five years of age, for a total of 14 points. Children were observed having free play, washing hands, eating morning snack, and the older children completed worksheets while the younger children listened to a story, practiced counting, and sang songs. Four children?s records, two staff records, and one household members? records were reviewed. The provider and assistant have current certification in CPR and First Aid. The provider does not administer medication and must have Medication Administration Training (MAT) certification to do so. The current week's menu was posted and morning snack today was strawberries and milk. The weather band radio was observed and in compliance. The emergency drill log and attendance were reviewed and were in compliance. The most recent evacuation drill was documented 1/29/20 and the most recent shelter-in-place was documented 8/21/19. Standard #: 22VAC40-111-100-A
Description: Based on review of four children's records, the provider did not obtain documentation of a physical examination by a physician within 30 days after the first day of attendance for one child. Evidence: The record for Child B, with a start date of 11/10/15, did not contain documentation of a physical.

Plan of Correction: I will ask the mother for that today.

Standard #: 22VAC40-111-60-B
Description: Based on review of four childrens records, the provider did not obtain all required documentation for each child's record. Evidence: The record for Child A did not contain written authorization for emergency medical care.

Plan of Correction: I will ask the parent to sign the form today.

Standard #: 22VAC40-111-70-A
Description: Based on review of four children's records, the provider did not ensure that parents received all required information before the child's first day of attendance. Evidence: 1. The record for Child C did not contain the provider's policy for administration of medication. 2. The record for Child A did not contain the provider's disclosure of whether or not she maintains liability insurance.

Plan of Correction: I will give this information to the parents today and have them sign the forms.
