Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today from 9:10am to 11:10am in conjunction with a monitoring inspection (see that inspection report for details). The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Two staff records were reviewed for documentation of the mandatory Virginia Preservice Training and current certification in First Aid and CPR. There are no children currently enrolled with diagnosed food allergies. The drill log was reviewed for documentation of an annual lockdown drill. Standard #: 22VAC40-665-230-B
Description: Based on review of two staff records, the provider did not ensure that all staff completed the required Virginia Preservice Training within the first 90 days of employment. Evidence: The record for Caregiver A, with a start date of 5/28/18, did not contain documentation of the Virginia Preservice Training. The provider stated that the caregiver has not yet completed the training.

Plan of Correction: I will ask her to start that today.

Standard #: 22VAC40-665-410-A-3
Description: Based on review of documentation and interview with the provider, lockdown drills were not practiced annually. Evidence: The drill log did not contain a lockdown drill and the provider stated that she and her assistant have discussed what to do for a lockdown, but have not completed a practice drill.

Plan of Correction: We will begin practicing this every year.
