Reminder: All staff are required to attend 16 hours of training annually.

Comments:

An unannounced monitoring inspection was conducted today between 9:00 am and 12:00 pm. There were 11 children present but only 10 were counted for capacity as one is the grandchild of the provider and lives in the home (31 points) with 3 caregivers. The children were observed during hand-washing and during diaper changing. The children were observed during breakfast, breakfast consisted of cereal, banana and milk. The children were also observed creating valentine cards for parents and listening to friends valentine wishes. The physical plant, attendance logs, evacuation logs, and plans, cribs, bottles, children's records, household member's records, caregiver's records, pet records, menu's and emergency supplies were inspected. There were no medications on site. Areas of non-compliance were cited on the Violation Notice. Standard #:	22VAC40-111-780-A
Description:	Based on review of children's records, the provider failed to obtain an annual acknowledgement of review of emergency contact information. Evidence: 1. Child #1's record did not have an annual review since 10/12/18. 2. Child #2's record did not have an annual review since 8/28/17. 3. Child #4's record did not have an annual review since 4/1/18.
Plan of Correction:	I will remember to update records annually.
Standard #:	22VAC40-111-100-A
Description:	Based on a review of children's records, the provider failed to obtain documentation of physical examinations by a physician by the 30th day of attendance. Evidence: Child # 2, #3 and #3's records did not have documentation of a physical by the 30th day of attendance.
Plan of Correction:	I will contact parents for information.
Standard #:	22VAC40-111-180-A
Description:	Based on a review of caregiver records, the provider failed to obtain for each caregiver a current report of Tuberculosis Screening for in accordance with the requirements that every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: Caregiver #1 and #2's Tuberculosis Test expired 8/20/19.
Plan of Correction:	Provider states she has documentation of current TB test and once she locates them she will forward them to Inspector.
