The form that identifies whether the provider administers medication must have the appropriate box checked so that parents are clearly informed.

Comments:
An unannounced renewal inspection was conducted from 11:15 am ? 1:15 pm with the provider and her assistant. There were 11 children in care, ranging from 1 year to four years of age, for a total of 26 points. The children were observed finishing lunch (which is brought from home), reading books, coloring, watching an age appropriate video, and napping. Four children's records were reviewed. Five provider/household member records were reviewed and in compliance. Required information was posted in addition to a variety of seasonal artwork. The last emergency escape drill was conducted: 2/3/2020 and the last shelter-in-place drill was conducted: 1/8/2020. Annual training was reviewed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Standard #: 22VAC40-111-100-A
Description: Based on a review of four children's records, the provider did not obtain documentation that each child has received a physical examination by or under the direction of a physician prior to the child's first attendance or within one month after attendance. Evidence: the records for child #1 and child #2 (date of first attendance: 1/6/20) did not have documentation of a physical examination.

Plan of Correction: Will ask the parent again to provide this documentation. Parent was emailed during inspection.

Standard #: 22VAC40-111-60-A
Description: Based on review of four children's records, it was determined that not all records contained all required information. Evidence: the records for child #1 and child #2 were missing work phone numbers for mom and dad, the addresses for two emergency contacts, and the medical insurance policy number. The records for child #3 and child #4 were missing the name, address and phone number for the second emergency contact.

Plan of Correction: Will follow up with all parents to obtain the missing information. Parents were texted during the inspection.
