Comments:
An unannounced monitoring inspection was conducted on 02/05/2020 from 9:35 am - 10: 45 am. At the time of entrance there were three children in care with the provider. The ages of the children counted for a total of 12 points. Children were observed playing in the child care room, eating snack and playing outside. Two diaper changes were reviewed.
Records were reviewed for three children in care in addition to the provider and one household member.
Violations were observed in five parts of the FDH standards and were reviewed with the provider at the conclusion of the inspection.
The violations are listed on the violation notice.

Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, the provider failed to ensure that bottles shall be labeled with the child's full name and the date, if more than one infant is in care.

Evidence:
1. The bottle for child 2 was not dated.
2. The bottle for child 3 was not dated and labeled with the child's name .

Plan of Correction: Child 2 is learning to drink from a cup at mealtime. Most of the time she want a bottle. I provide whole milk for her at mealtime. Mom bring bottle with her name but not date.

Standard #: 22VAC40-111-960-G
Description: Based on observation, the provider failed to ensure that bottles shall not be heated in a microwave oven.

Evidence:
The bottles of child 2 and child 3 were heated up in the microwave oven located in the kitchen.

Plan of Correction: Once provider was reminded not to warm child's milk in microwave oven. I changed to a pot to warm milk.

Standard #: 22VAC40-111-980-B
Description: Based on record review and interview the provider failed to ensure that special written permission shall be obtained from the parent of each child for the provider to take the child on special field trips (those not regularly scheduled). The written special permission shall specify destination, duration of trip, and driver, if the child is to be transported.

Evidence:
1. There was no written permission from the parent of child 1, child 2 and child 3 for a field trip to a local park on the morning of 02/04/2020.
2. The provider confirmed that she had not obtained written permission from each parent for the field trip on 02/04/2020.

Plan of Correction: All field trips will have appropriate permission forms.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that children's records contained all of the required elements.

Evidence:
1. The enrollment record for child 1 lacked documentation of the first date of attendance.

Plan of Correction: Child's mother completed the first date of attendance.

Standard #: 22VAC40-111-90-C
Description: Based on record review the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1. The most recent immunization documentation on file for child 2 were administered on 05/24/2019.
a. Child 2 is under the age of two years and was in care during the inspection.

Plan of Correction: Child 2 immunization record is in her file.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider failed to ensure that child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

Evidence:
1. One outlet in the child care room lacked a protective cover.
a. Children were observed in this room during the inspection.

Plan of Correction: All outlets are covered.

Standard #: 22VAC40-111-320-E
Description: Based on observation, the provider failed to ensure that portable electric heaters, and similar heating devices shall be located at least three feet from combustible materials.

Evidence:
1. An electric radiator space heater was observed in use in the child care room. The heater was turned on and was surrounded by equipment and furniture.
a. The heater had been placed up against a wall and was enclosed on three sides by a metal crib, a metal storage shelf and a stack of two children's cots.
b. A cotton sheet and three stuffed animals were placed on top of the cots.
2. The warning label on the heater stated: "Do not place any objects such as furniture, papers, clothing any closer than 3 feet to the front of the heater and keep away from the sides and rear when the heater is engaged."
3. Children were observed in the child care room during the inspection.

Plan of Correction: The electric heater has been removed.

Children are never in the playroom without an adult.

There are no cots stored in daycare room.

Standard #: 22VAC40-111-320-G
Description: Based on record review and interview, the provider failed to ensure that wood burning stoves and fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider.

Evidence:
1. The most recent inspection of the fireplace and chimney was conducted on 11/12/2018.
2. The provider confirmed this was the most recent inspection for the fireplace and chimney.

Plan of Correction: The stove will be clean as soon as possible.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface shall be cleaned and sanitized after each use.

Evidence:
1. The provider did not clean and sanitize the diapering surface after changing the soiled and wet diaper of child 2 at approximately 9:40 am.
2. After completion of the diaper change the provider put two paper towels on the diapering surface and then placed child 1 on the diapering surface for a diaper change.

Plan of Correction: I cleaned the surface once the child was in a safe place. With soap/water and bleach water.

Standard #: 22VAC40-111-690-E
Description: Based on observation, the provider failed to ensure that soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

Evidence:
1. The provider used her hands to open the lid of the diaper pail three times when she disposed of the soiled diaper, wipes and gloves for a diaper change for child 2.
2. The provider used her hands to open the lid of the diaper pail twice when she disposed of the wet diaper and wipes for a diaper change of child 1.

Plan of Correction: Soiled disposable diapers and wipes are disposed of in a leak proof plastic lined diaper container that open by foot. Provider was unable to use her feet at that time because the baby was getting up so I quickly used my right hand with the glove to open lid and I removed the glove. without touching the child.
