Comments:
An unannounced renewal inspection was conducted on Monday, December 30, 2019 to determine compliance with licensing standards. The inspector arrived at 12:16 PM and concluded the inspection at 2:08 PM. Today, there were eight children, totaling 20 points, present with two caregivers. During the inspection, children were napping, taking bathroom breaks and reading stories. The provider stated that medication is not administered at this time. The provider stated transportation is not provided at this time. Children?s equipment, learning materials, indoor and outdoor space, restroom, first aid kit and the nonmedical emergency supplies were inspected. Five children's records, two caregiver records, one household record, daily attendance records, fire drill and shelter in place drill logs and all required postings were reviewed.

The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and are documented on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-111-60-A
Description: Based on review of children's records, the center did not ensure that each child's record contains all required information.

Evidence:
1. The record for child #4 did not contain the address for the two of the designated people to call in an emergency.
2.The record for child #5 did not contain the address for one of the two designated people to call in an emergency.

Standard #: 22VAC40-111-230-A
Description: Based on a caregiver record review and interview, the provider did not maintain documentation of each caregiver's annual training.

Evidence:
1. The caregiver's record did not contain documentation of annual training hours.
2.The caregiver stated she did not have documentation of training and she was unaware of the annual 16 clock hours of training relevant to their job responsibilities.

Description: Based on observation, the provider did not ensure that buckets accessible to children shall be emptied immediately after use.
Evidence:
In the kitchen, a bucket contained liquid and a mop. The bucket was located on the floor next to the children's bathroom.

Description: Based on observation, the provider did not ensure potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.

Evidence:
1. A container of Clorox wipes labeled "Harmful" and "Hazardous to humans" was on the kitchen counter.
2. A bottle of Windex labeled "Danger" and "Keep out of reach of children" was on the bathroom counter.

Standard #: 22VAC40-111-530-G
Description: Based on observation and interview, the provider failed to ensure that children under two years of age did not use pillows.
Evidence:
1. In room#1, one infant in a crib was sleeping on top of a Boppy pillow.
2. The provider reported that the infant's parent requested use of the Boppy pillow.
