Comments:
An unannounced renewal inspection was conducted today at approximately 10:15am. There were 5 children (11 points) directly supervised by the provider. The home, 2 staff records, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and learning the alphabet in English and Spanish . Toileting procedures were also observed. There was an adequate supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Description: Based on review of Child records it was determined that all required information was not maintained.

Evidence: During inspection Child 1 and 3 were missing that workplace and number for parents listed in file.

Plan of Correction: I will get the information from parents. I think they only use their cell phones
