Technical Assistance:
The provider was reminded that the trampoline could not be used by daycare children; and should not be used while these children are present.

Comments:
An unannounced monitoring inspection was conducted from 10:30 am to 11:50 am. There were 3 children (6 points) in direct are and supervision of 2 caregivers. The children were observed playing with toy cars, coloring pictures and writing the letters of the alphabet. 3 children's records, 2 caregiver records and 2 household member records were reviewed. Handwashing was observed lunch. There were no medications on site. Emergency drill records were current. Standard #: 22VAC40-111-240-A
Description: Based on observation, outside equipment of the family day home was not maintained in a safe condition.
Evidence: The round slide on the playground had pieces that were broken and coming loose, creating a gap between two parts of the slide that could pose an entrapment hazard.

Plan of Correction: The children are not using the slide. I will have someone repair the slide and until then the children will not be allowed to use the slide.
