Comments:
An unannounced monitoring inspection was conducted today from approximately 3:40pm-5pm. There were 12 children (35 points) in the direct care and supervision of 2 caregivers. A sample of 6 children?s records, 2 caregiver's records and 5 household member's records were reviewed. The children were observed reading books, playing with toys, eating snack and being picked up by their parents. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider failed to review with the parents each child's file at least annully.

Evidence:
1. Child A's file was last reviewed 02/04/2018.
2. Child C's file was last reviewed 05/06/2018.
3. Child D's file was last reviewed 09/08/2018.

Plan of Correction: Parents will review files today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were missing required information.

Evidence:
1. Child A was missing the mother's employer phone number, first day of enrollment, and information for parents.
2. Child B was missing one of two required emergency contacts.
3. Child C was missing the name and phone number of one emergency contact, information for parents, provisions of emergency preparedness, and medication policy.

Plan of Correction: Children's files will be reviewed and corrected today.

Standard #: 22VAC40-111-170-A
Description: Based on record review, the provider failed to obtain an initial tuberculosis screening for a household member.

Evidence:
Household Member A did not have an initial tuberculosis screening on file.

Plan of Correction: He will get it done next week.

Standard #: 22VAC40-111-280
Description: Based on observation, a poisonous substance was in reach of the children.

Evidence:
Windex was located in an unlocked cabinet below the sink in the bathroom.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-570-A
Description: Based on record review and observation, the provider failed to ensure that a caregiver did not exceed 16 points.

Evidence:
Caregiver A and B were caring for 12 children 35 points at the time of inspection.

Plan of Correction: I usually have additional help. They were not here today.
