Technical Assistance:
Licensing inspector discussed provider attendance and inspection attempt expectations with provider.

Comments:
An unannounced monitoring inspection was conducted today from approximately 2:15pm-4pm. There were 8 children (24 points) in the direct care and supervision of 2 caregivers. A sample of 4 children?s 2 caregiver's records and 1 household member's record were reviewed. The children were observed napping, and watching television. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-50-A
Description: Based on record review, the family day home failed to keep a written record of daily attendance.

Evidence:
Attendance for November, December, and 01/23/2020 were not documented.

Plan of Correction: I will keep my attendance.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's files were missing required information.

Evidence:
Child A was missing father's work phone number, and insurance policy number.
Child B was missing insurance policy number and the name, address and phone number of 1 of 2 required emergency contacts.

Plan of Correction: Parents will update files today.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas of the home were not kept in an operable condition.

Evidence:
The doorbell to the main child care area did not work.

Plan of Correction: I will get it fixed.
