An unannounced monitoring inspection was conducted today from approximately 10am-12:15pm. There were 3 children (9 points) in the direct care and supervision of 1 caregivers. A sample of 3 children?s records and 1 caregiver's record and 1 household member's record were reviewed. The children were observed napping, and eating. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider failed to obtain a physical examination for each child in care.
Child A and Child B did not have a physical exam on file.
Plan of Correction: I will ask parents for the exam.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were missing required information.
Child A was missing information for parents.
Child C was missing information for parents and provisions of emergency preparedness.
Plan of Correction: Parents will update files today.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider failed to obtain subsequent tuberculosis screenings for caregivers and household members.
The tuberculosis exams for Caregiver A and Household Member A were last conducted 12/12/2017.
Plan of Correction: I will do it this weekend.
