63.2 Licensure and Registration Procedures

Please refer to the public website to obtain the staff model forms for orientation and emergency training.
You are reminded that a total of 16 hours of annual training is required annually.
You have children in care under the age of 2 years, please be sure that updated immunizations are obtained every 6 months. Documentation will be required by the end of this month for some children currently in care.
An unannounced monitoring inspection was conducted this date. A total of 12 children in care (20pts) with 3 Assistants present. The Provider was not present today. When the Inspector arrived, the children were dancing to songs along with staff. The younger children were observing the older children during this gross motor time. A nice transition observed where the younger children were in a circle while one staff read books to them. The older children sat at the table and reviewed the days of the week, weather, and the letter "Q". They completed a worksheet of the letter "Q" and took turns calling out words that began with the letter "Q". Hand washing procedures, proper sanitizing of surfaces such as table and high chair trays were observed. The children were excited for "pizza Friday". Lunch today was pizza, spinach, peaches and milk to drink. Staff sat with the children while they ate and assisted those that needed help. Ample supply for second servings. A total of 6 child records and all staff records were reviewed today. Two repeated violations from the last inspection found. This inspection began at approximately 9:15am and ended at 12:30pm. Standard #: 22VAC40-111-750-A Description: Based on review of all topical ointments on site, authorizations were not obtained.
Evidence: Four topical ointments for Child #3, #4, #5, and #6 did not have written parent authorizations for use.
Plan of Correction: I will either return the topical ointments to the parents if they are no longer needed or will obtain a written authorization to use them.
Standard #: 22VAC40-111-100-A Description: Based on child record review, one health examination was not obtained within 30 days of enrollment.
Evidence: Child #3 began care on 9/9/19. A physical examination was not on file for this child.
Plan of Correction: I will request the health examination from the parent.
Standard #: 22VAC40-111-120-B Description: Based on staff record review, one staff record was incomplete.
l. Staff #4's record did not contain the following required information: start date, two references, emergency contact information, documentation of orientation and emergency training.
Plan of Correction: Provider was not present during the inspection but spoke to the Inspector by phone. All information was obtained and completed but needs to printed and maintained. Will print all documents when Provider returns.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the findings for the most recent inspection was not maintained.
Evidence: The findings from the September 2019 inspection was not available/posted.
Plan of Correction: We will post today's inspection.
Standard #: 63.2-1720.1-B-2 Description: Based on staff record review, one background search result was not found.
Evidence: Staff #4's record did not contain the results of a fingerprint background check.
Plan of Correction: This staff completed the fingerprint background search. We will call Office of Background Investigations to inquire the status of the search. Once received we will forward the results to the licensing inspector within 10 days.
