Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the renewal process.

Comments:
The following inspection took place between the hours of 2 pm through 4 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Lovettsville, Virginia. At the start of this unannounced renewal inspection there were a total of 7 children observed (19 points) with 2 caregivers. A total of 5 children?s files, 2 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: Nap time, story time, diapering, hand washing, snack time, movie time, games, learning, and other organized activities. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 5 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the child's first day of attendance.

Evidence -
1. On the date of inspection (02/21/2020), documentation of a physical for Child #5 was not available for review.
2. The start date for Child #5 was listed as 01/06/2020. Child #5 was in attendance of the date of inspection.

Plan of Correction: Documentation of a physical examination shall be obtained and placed on file for Child #5.

Standard #: 22VAC40-111-90-C
Description: Based on review, 1 of 5 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (02/21/2020), the most current immunization page on file for Child #1 was dated - 05/03/2019.
2. Child #1 was under the age of two years on the date of inspection.

Plan of Correction: The parent of Child #1 shall be contacted and updated information shall be obtained.
