Technical Assistance:
Discussed with the Provider what needs to be documented for shelter-in-place drills and how capacity and points are counted in regards to household children.

Comments:
An unannounced renewal inspection was conducted today from 9:05am to 12:00pm with the provider. There were four daycare children and one household child in care, ranging from six months to two years of age, for a total of 15 points. Children were observed having free play, reading books and discussing them with the provider, singing songs, having diapers changed, and laying down for a nap. Four children?s records, the provider's record, and one household member's record were reviewed. The provider has current certification in CPR and First Aid. One medication and authorization form were reviewed and the provider has current certification in Medication Administration Training (MAT). Lunch today will be lentils, orzo, pineapple, mixed vegetables, and milk. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed and were in compliance. The emergency drill log, attendance, and menu were also reviewed. Standard #: 22VAC40-111-830-C
Description: Based on review of documentation and interview with the provider, the provider did not maintain documentation of shelter-in-place drills. Evidence: The emergency drill log did not contain documentation of any shelter-in-place drills and the provider stated that she did complete the drills but did not realize she was supposed to document them.

Plan of Correction: I will do one tomorrow and will document it. I will document two per year moving forward.

Standard #: 22VAC40-111-60-A
Description: Based on review of four children's records, the provider did not obtain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached for all children. Evidence: The record for Child A did not contain the house number and street name for one emergency contact, and did not include a second emergency contact.

Plan of Correction: I will ask the parents for this information again today.

Standard #: 63.2-1721.1-A
Description: Based on documentation review, the provider did not obtain repeat central checks every five years as required. Evidence: The most recent central registry checks on record for the provider and Household Member A were dated 7/28/14.

Plan of Correction: I am sure I have completed them more recently but I could not find them during the inspection. I called the Central Registry office during the inspection to see if they can send me copies of a more recent check. I will also complete new checks today if they are unable to send me a more recent copy.
