54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Provided consultation on:
1) Ensure that paint in the downstairs area is not chipping or peeling
2) Ensure that the gate preventing access to children under age two is able to be securely fastened
3) Hard foods are considered a choking hazard and may not be served to children under age four.
4) Ensure that all child records match the legal name of the child as listed on the birth certificate

Comments:
An unannounced monitoring inspection was conducted from 11:00 am - 2:00 pm with the provider and her assistants. There were 12 children in the care of three providers, ranging from six months to six years of age, for a total of 29 points. The children were observed having free play, watching an age-appropriate video, eating lunch and napping. Lunch served today: spaghetti with chicken and tomato sauce, and apples. Eight children's records and four provider/household member records were reviewed. The last emergency escape drill was conducted: 2/5/20, the last shelter-in-place drill was conducted: 1/20/20 and the last lockdown drill was conducted: 4/10/19. The provider is Medication Administration Training (MAT) certified but there was no medication to review. Violations:
Standard #: 22VAC40-111-880
Description: Based on observation, and provider interview, it was determined that not all meals provided by the family day home followed the most recent, age-appropriate nutritional requirements of the USDA. Evidence: the younger children were not provided anything to drink with lunch and the older children were served water with their lunch.

Plan of Correction: Will serve milk with all meals.

Standard #: 22VAC40-111-100-A
Description: Based on a review of eight children's records, the provider did not obtain documentation that each child has received a physical examination by or under the direction of a physician prior to the child's first attendance or within one month after attendance. Evidence: record for child #8 (date of first attendance: 10/23/19) did not have documentation of a physical examination.

Plan of Correction: Called mom during inspection - will obtain documentation of the physical this week.

Standard #: 22VAC40-111-50-A
Description: Based on review of the written attendance record and observation, it was determined that the provider did not ensure all children were accurately included on the attendance report. Evidence: Ten children were listed on the attendance record however the LI observed 12 children in care.

Plan of Correction: Provider corrected the attendance during the inspection.

Standard #: 22VAC40-111-70-A
Description: Based on review of eight children's files, the provider failed to have documentation that all policies were provided to parents in writing before the first day of attendance. Evidence: The records for child #1, child #2, and child #4 did not have signed documentation that parents had been given VDSS model form "Information for Parents". The record for child #7 had a signed form, however the form was blank, and did not have any information for parents listed.

Plan of Correction: Will provide parents with the completed model form to obtain signatures today.

Standard #: 22VAC40-111-480-B
Description: Based on observation, not all equipment and materials used by a child were age appropriate. Evidence: A 19 month old child was observed strapped into an infant swing.

Plan of Correction: Will not put him in a swing at day care.

Standard #: 22VAC40-111-510-A
Description: Based on observation, and provider interview, not all children were provided an individual crib for resting or napping. Evidence: A six month old baby was put in a swing to nap and a ten month old baby was observed sleeping in a swing. The provider stated that the babies sometimes are put to sleep in their crib.

Plan of Correction: From now on babies will be put to sleep in their cribs.

Standard #: 22VAC40-111-530-F
Description: Based on observation, soft bedding was used around infants. Evidence: a ten month old baby was observed in a swing sleeping with a heavy blanket on top.

Plan of Correction: Blanket was removed during inspection. Blankets will not be used under or around infants.
