Technical Assistance:
Discussed arrangement of classroom and future use of playpens. Referred Provider to 22VAC40-111-550.

Comments:
An unannounced renewal inspection was conducted today between approximately 10:05am and 1:15pm. 11 children (31pts) were present with 2 caregivers. Ratios were in compliance. The physical space, 3 children's records, Provider/Assistant/household member records, the Emergency Preparedness Plan, the menu, emergency supplies, evacuation drills and required posted documents were reviewed. No medications were on site. Annual training hours for both the Provider and Assistant were in compliance. Children were observed playing with age appropriate materials. Interactions between the caregivers and children were positive. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-50-A
Description: Based on record review and interview with the Provider, the family day home did not have a current written record of children in care.
Evidence: The Provider could not produce a written record of attendance for the past 2 weeks.

Plan of Correction: I will start keeping a record of attendance again.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 2 out of 3 records had an incomplete Liability Insurance form.

Evidence: The Liability Insurance form for Child #1 and Child #2 was not signed by the parent.

Plan of Correction: I will ask the parents to sign the forms right away.

Standard #: 22VAC40-111-180-A
Description: Based on record review, subsequent TB tests had not been obtained for 5 out of 9 household members.
Evidence: Household members #1, #2, and #3 all expired on 10/20/2019. Household members #4 and #5 expired on 10/19/2019.

Plan of Correction: I will have everyone get updated TB screenings.
