Comments:
An unannounced monitoring inspection was conducted today from 11:30am-12:30pm. There were 3 children (10 points) directly supervised by 2 caregivers. The home, 2 staff records, 3 children?s records, 6 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, being held while bottle feeding and beginning to take a nap. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Description: Based on observation, bottles were not labeled with the child's full name and date.

Evidence: Two bottles observed on a high chair did not have a name or a date.

Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
