The following inspection took place between the hours of 10 am through 12:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Sterling Park, Virginia. At the start of this unannounced renewal inspection there were a total of 4 children observed (8 points) with 1 caregiver. A total of 4 children?s files, 1 staff file, and 2 household member?s files were reviewed. The children were engaged in the following: drop off, games, music, song, dance, toy play, bathroom breaks, hand washing, lunch, nap time, and other organized activities. If you should have any future questions, please call or send an e-mail to charles.perkins@dss.virginia.gov. Thank you.

Violations:
Standard #: 22VAC40-111-210-A
Description: Based on review, 1 of 3 children files indicated that in addition to satisfactory completion of first aid training and CPR training, a caregiver did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence -
1. On the date of inspection (02/25/2020), Staff #1 had a total of 12 hours of training obtained in the 2019 annual training year.
2. These documents were viewed on file within the Family Day Home (FDH).

Plan of Correction: A total of 16 annual training hours shall be obtained within the current 2020 training year.

Standard #: 22VAC40-111-500-A
Description: Based on observation, a non-climbable barrier (at least four feet high such as, but not limited to, a fence) did not surround the outdoor play area (back yard) located near a street.

Evidence -
1. On the date of inspection (02/25/2020), an inspection was conducted of the new location for the Family Day Home located at 117 Kennedy Road, in Sterling, Virginia.
2. While the play area located in the back yard of the home does have fencing on the on the side and back of the play area; the area between the house and on either side of the outdoor storage shed has been left open. A child could travel from the backyard to the front yard, and then access the street without any barrier.
3. This open area is located on the left side of home (as one faces the residence). The right side of home has a gate. Currently, the Family Day Home has yet to start at this new location.

Plan of Correction: A barrier will be installed on both sides of the storage shed in order to prevent children access to the front yard and street.
