Comments:
An unannounced renewal inspection was conducted on 02/25/2020. The inspection began at 12:00 PM and concluded at 2:30 PM. There were three children in care with a total of 11 points. The provider and substitute provider were present on this day and during the inspection. Children were observed during free play, nap, and media time. The inspector reviewed all four children's records. The inspector reviewed provider and household members' records. The inspector examined rooms used for care, drill documentation, and the emergency supplies. This provider does not administer medications. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation and discussion with the substitute provider, the provider failed to ensure that bottles shall be labeled with the child's full name and date, if more than one infant is in care.

Evidence:

1. The LI observed two bottles located near the kitchen sink which did not have dates or names.
2. The LI discussed the violation with the substitute provider and they acknowledged that more than more child drinks a bottle.

Plan of Correction: The provider will label the bottles with name and date everyday.

Standard #: 22VAC40-111-320-B
Description: Based on observations, the provider failed to ensure that child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

Evidence:

Approximately five unused outlets were not covered with protective covers in the upstairs living room. Children were located in the upstairs living room during the inspection.

Plan of Correction: The provider will ensure that if the upstairs living room is used, outlet covers will be installed.
