Technical Assistance:
The license to operate a family day home is due to expire on 3/26/2020. As of today the Fairfax Licesning Office has not recieved your renewal application. Applications must be recieved no later than 30 days prior to expiration. PLease complete and submit your application as soon as possible.

Please note that some topical ointments are considered medications and should by treated as such. Topical ointments excluding: diaper rash cream, sunscreen, bug spray, and teething gel should only be administer with the appropriate parent and physician authorizations. Administering these medications should also follow your disclosed medication policy.

Comments:
An unannounced monitoring/renewal inspection was conducted today from 10:00 am-12:00 pm. There were 5 children (14 points) directly supervised by the provider and substitute provider. The home, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. Diapering and hand washing procedures were also observed. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation, medication was not kept a locked place using a safe locking method that prevents access by children.
Evidence:
Two tubes of anti fungal ointment were observed on the top of the toilet tank in the bathroom used by children.

Plan of Correction: I am sending the medication home with the family today.

Standard #: 22VAC40-111-750-A
Description: Based on observation, when topical ointments were used all requirements were not met.
Evidence:
1. Two tubes of diaper rash cream were observed without a label indicating the owner.
2. No written authorizations from the parent were available for review on the date of inspection.

Plan of Correction: I will move the diaper rash cream and label the tubes.

Standard #: 22VAC40-111-750-B
Description: Based on observation, topical ointments were not stored in an area inaccessible to children.
Evidence:
Two tubes of diaper rash cream were observed on top of the toilet tank in the bathroom used by children.

Plan of Correction: I will move the cream to a higher location.

Standard #: 22VAC40-111-830-B
Description: Based on record review and provider statement, shelter-in-place drills were not practiced at least twice per year.
Evidence:
Shelter in place drills were not documented for 2019. The provider stated that only evacuation drills were completed.

Plan of Correction: I will complete shelter in place drills for 2020.

Standard #: 22VAC40-111-100-A
Description: Based on record review, one child enrolled for longer than 30 days did not have documentation of a physical examination by or under the direction of a physician on file.
Evidence:
Child 4 (Enrolled: 7/15/2019) does not have the results of a physical examination file.

Plan of Correction: I will ask the parents to provide a physical.

Standard #: 22VAC40-111-60-B
Description: Based on record review, three of five children's records reviewed were incomplete.
Evidence:
1. Child 1's record did not contain the parent's work phone number.
2. Child 4's record did not contain the results of a health examination.
3. Child 5's record did not contain the name of the mother's employer or her work phone number.

Plan of Correction: I will ask the parents to provide the missing information.
