Technical Assistance:
Discussed crib requirements and labeling infant bottles.

Comments:
An unannounced monitoring inspection was conducted today from 11:30am-2:00pm. There were 3 children (8 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing with blocks, being bottle fed, and playing freely. There was an adequate supply of books, toys, and materials for the children. Areas of non-compliance are identified on the Violation Notice.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain a physical examination by or under the direction of a physician within 30 days of attendance for a child in care.

Evidence:
Child#1's record does not contain a physical examination. The child's first day of attendance was documented as 9/30/2019.

Plan of Correction: I will ask the parents to bring a physical examination for the child.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete.

Evidence:
Child#1's record is missing 1 of 2 required emergency contacts, the child's medical insurance policy number, authorization for emergency medical care. Child#2's record is missing the father's work phone number. Child#3's record is missing the parents' work phone number, 1 of 2 required emergency contacts, and the child's emergency medical insurance policy number. Child#4's record is missing the authorization for emergency medical care.

Plan of Correction: I will talk to the parents to obtain the missing information.

Standard #: 22VAC40-111-80-A
Description: Based on record review, the provider did not obtain from the parent within seven business days a proof of age and identity for a child in care.

Evidence:
Child#4's record does not include documentation that the child's proof of age and identification was viewed within 7 business days of attendance. The child's first day of attendance was documented as 2/1/2020.

Plan of Correction: I will ask the parents to bring it to me as soon as possible.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider did not obtain documentation of additional immunization once every six months for a child in care.

Evidence:
Child#1's record contains an immunization record that was last updated on 6/1/2018.

Plan of Correction: I will request a new immunization record.

Standard #: 22VAC40-111-210-A
Description: Based on record review, a caregiver did not obtain a minimum of 16 clock hours of annual training in areas relevant to their job responsibilities.

Evidence:
Staff#2 has only completed a total of 10 hours of the 16 hours of annual training required during the preceding licensing period.

Plan of Correction: I will ensure 16 hours of training classes are completed before September 2020.

Standard #: 22VAC40-111-240-A
Description: Based on observation and interview, areas and furnishing of the Family Day Home were not maintained in a safe, and operable condition.

Evidence:
One of two cribs in the Family Day Home was being used without documentation that it meets current U.S. Consumer Product Safety Commission (CPSC) standards. The standard requires a label on the crib that indicates that the crib was manufactured after June 28, 2011, or a certificate of compliance indicating that the crib meets federal standards. The crib observed during the inspection has a label that is no longer showing the date of manufacture, and the provider doesn't have the registration card required to accompany the crib.

Plan of Correction: We will buy new cribs and mats within the next two weeks.
