An unannounced renewal inspection was conducted today between the hours of 9:45am through 11:00am with the provider on site. There were 5 children (15 points) with the provider. A sample of 4 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, singing songs, reading stories with the provider, bottle feeding and diaper changes. Interactions between the children and provider were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 (Start date 12/02/2019) and Child #2 (Start date 1/06/2020) did not have documentation of a physical examination in their files.Plan of Correction: I will ask the parents for the documentation today.
