Technical Assistance:
Technical assistance was offered regarding pets and training hours.

Comments:
An unannounced renewal inspection was conducted from 10:00am through 11:50am. There were 9 daycare children (20 points) and 4 resident children with 2 caregivers present. 5 children's records and 2 caregiver's records were reviewed. The children were observed in the following activities: playing with toys, completing worksheets, bathroom breaks, hand washing, and getting bottle fed. A walk through of the family family day home was conducted. Emergency drill records and supplies were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles for infants were not labeled with the child's name and dated.
Evidence:
1. There were three infants in care.
2. A milk bottle for Child # 1 was not labeled with her name and date.

Plan of Correction: The bottles will be labeled and dated.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain a current report of Tuberculosis (TB) Screening form for each caregiver.
Evidence: Caregiver # 1's most recent TB screening on 2/12/20.

Plan of Correction: The TB screening will be obtained.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas inside the family day were not maintained in a safe condition.
Evidence:
1. A wall pillar in the daycare was stripped of the paint and the dry wall was exposed.
2. There were several areas of the drywall that were breaking and tearing away, and had holes.

Plan of Correction: The dry wall will be covered until it gets fixed which will be by Monday.

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom did not have paper towels.
Evidence: A child was observed using the bathroom and washing hands after. There were no paper towels in the bathroom for the child to use.

Plan of Correction: Paper towels will be placed in the bathroom.

Standard #: 22VAC40-111-460-I
Description: Based on observation, animal food and water dishes were accessible to children.
Evidence:
1. Water and food dishes for the provider's dogs were found in the kitchen on the second level.
2. 5 children were in the dining area next to the kitchen, and the dishes were accessible to the them.

Plan of Correction: The food and water dishes were removed from the kitchen to another part of the home.
