Discussion was held on the topics of current enrollment, and staffing.

Comments:
The following inspection took place between the hours of 10 am through 12 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Arlington, Virginia. At the start of this unannounced remonitoring inspection there were a total of 5 children observed (13 points) with 1 caregiver, and 1 volunteer. A total of 5 children?s files, 1 staff file, and 1 household member?s file was reviewed. The children were engaged in the following: toy play, games, learning, music, song, diapering, bathroom breaks, hand washing, lunch, nap time, and other organized activities. Violations:
Standard #: 22VAC40-111-780-B
Description: Based on review, 2 of 5 children files indicated that annually, the Provider did not review with the parent the emergency contact information (required in 22 VAC 40-111-60 B 2) to ensure the information was correct, and obtain the parent's signed acknowledgment of this review.

Evidence -
1. On the date of inspection (02/27/2020), the current review on file for Child #2 was dated - 11/18/2018.
2. The current review on file for Child #3 was dated - 01/01/2019.

Plan of Correction: The parents of all applicable children shall be contacted and the information reviewed, (if needed) updated, and signed (and dated) for verification.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface was absorbent.

Evidence -
1. On the date of inspection (02/27/2020), the changing pad was viewed with a small rip upon its surface.
2. The rip exposed the padding underneath, thus rendering it no longer non-absorbent.

Plan of Correction: Corrected on the date of inspection. The Provider had another changing pad (not in use) and used it to replace the one that was ripped.
