Technical Assistance:
Discussed requirements regarding children's immunizations.

Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:10 am through 11:00 am with the provider on site. There were 10 children (23 points) with the provider and one caregiver. A sample of 5 children's files and 4 caregiver/household member's files were reviewed. The children were observed engaged in imaginative play and playing with playdough. The children also actively participated in interactive story time. Handwashing and snack time was also observed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on record review, the provider did not obtain documentation that one child was adequately immunized before the child began attending the family day home.

Evidence:
Child #1 began attending the family day home on 10/10/19. Documentation of immunizations for Child #1 were obtained on 10/28/19.

Plan of Correction: I will ensure immunizations are collected before their first day.
