Comments:
An unannounced monitoring inspection was conducted today from 10:40am-1:00pm. There were 12 children (39 points) supervised by 3 direct care staff. The home, 2 staff records, 6 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely and eating lunch. Lunch served was deli meat, macaroni with tomato and mushroom sauce, oranges, and milk. Handwashing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with the child's full name and the date.

Evidence:
1. Child #1 (age: 2 years old) had 2 bottles that did not document the child's name and date.
2. Child #2 (age: 16 months old) had 1 bottle that did not document the child's last name and date.
3. Child #3 (age: 10 months old) had 1 bottle that did not document the date.
4. Child #4 (age: 11 months old) had 2 bottles that did not document the date.
5. Child #5 (age: 3 months old) had 1 bottle that did not document the date.

Plan of Correction: Names and dates were added to bottles during inspection.

Standard #: 22VAC40-111-530-F
Description: Based on observation, the provider did not ensure that no soft bedding of any kind shall be used under or around infants.

Evidence: Child #5 (age: 3 months old) and Child #12 (age: 7 months old) were lying in their cribs with loose soft blankets on top of them.

Plan of Correction: Blankets were removed during inspection.

Standard #: 22VAC40-111-680-A
Description: Based on observation, caregivers did not wash their hands with liquid soap and warm running water.

Evidence:
1. Caregiver C (date of hire: 9/21/2017) cleaned a child's nose and rinsed their hands with water.
2. Caregiver B (date of hire: 11/29/2016) and Caregiver C began feeding infants and did not wash their hands prior to feeding.

Plan of Correction: I will remind caregivers not to panic when inspector comes and to always wash their hands.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregivers did not ensure that children's hands are washed with liquid soap and warm running water.

Evidence: The children in the younger infant room were placed in chairs and were fed without having their hands washed prior to feeding.

Plan of Correction: I will remind caregivers not to panic when inspector comes and to always wash children's hands.
