Comments:
An unannounced monitoring inspection was conducted today from 11:40am-1:30pm. There were 7 children (27 points) supervised by 2 direct care staff. The home, 3 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed singing songs with musical instruments, having storytime, and eating lunch. Lunch served was french toast, carrots, blueberries, chicken, and milk. Handwashing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, shelter-in-place procedures were not practiced a minimum of twice per year.

Evidence: There was one documented shelter-in-place drill for 2019.

Plan of Correction: We will do one tomorrow.

Standard #: 22VAC40-111-60-B
Description: Based on record review, a child's record did not contain the following information:

Evidence:
1. 1. Child #2's (age: 19 months) record did not contain documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. The most recent documented review was dated 12/9/2018.
2. Child #5's (8 months) record did not contain documentation of the child's medical insurance policy number.

Plan of Correction: We will have the parents update tonight.

Standard #: 22VAC40-111-70-A
Description: Before the child's first day of attendance, parents were not provided in writing the following information:

1. Child #3's (age: 3 months) record did not contain documentation of whether or not there is liability insurance.
2. Child #5's (age: 8 months) record did not contain documentation of the parent's written acknowledgement of the receipt of the information in this section.

Plan of Correction: I will have the parents sign them.

Description: Based on record review, the provider did not obtain for each caregiver and adult household member a current report of TB screening form every two years from the date of the first screening.

Evidence: Household member A's most recent TB screening was dated 2/6/2018 (expired: 2/6/2020).

Plan of Correction: I will have them make an appointment.
