Technical Assistance:
Diaper changing procedures and disposal were discussed with the provider.

Comments:
An announced initial inspection was conducted today. The inspector arrived at 10:00 AM and departed at 12:00 PM. Four children, totaling eleven points were in care with two caregivers. The entire home, inside and outside, was inspected. The children spend the day in two rooms in the home to which there is an adjoining bathroom. There is a sun room used on rainy days and a fenced outdoor area. Five caregiver/household records and four children's records were reviewed. Morning activities and lunch service were observed.

An exit meeting was conducted with the provider prior to closure of the inspection.

Violations:
Standard #: 22VAC40-111-240-A
Description: Based upon observation, the provider has not ensured that areas outside of the home are safe.
Evidence:
1. There is a five inch metal spike protruding from the ground about four inches out from the exterior wall of the home by the back door. A child could fall upon the spike and be injured.
2. There is a ten inch black coated wire extending from the back of the house. The type of wire or the connection is unknown.

Plan of Correction: The provider responded with the following:
If the spike cannot be removed, it will be covered with cushioning or foam.
The wire will be cut to be flush with the wall.

Description: Based upon review of five household/caregiver records, the provider has not ensured that results of a child protective services central registry check is obtained are obtained from all states in which a person has lived within the past five years.
Evidence:
A CPS central registry search for a state other than Virginia in which household member 3 has lived has not yet been obtained.

Plan of Correction: The provider responded with the following:
A CPS central registry search will be obtained for household member 3.
