Technical Assistance:
We discussed the requirements for children's records, resident records, background checks, outside equipment, and hot water at faucets. The risk assessments for violations were provided.

Also, we reviewed requirements for knife storage, medication storage, hand washing and electric outlets.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 11:43 AM to 5:35 PM. Today, there were three children in care during the day, ages nineteen months through three years (7 points) with the provider. Two school age children attended after school. I viewed daily routines, staffing, supervision, interactions with children, activities, equipment, posted information, emergency supplies and drill records, menu, nutrition, five records for children three records for caregivers/residents, caregiver training, chimney inspection, inside and outside areas. There were frequent positive interactions with individual children. Today, the children enjoyed active play outside. A variety of toys, learning materials and age appropriate equipment were available. The provider used a book activity for learning how to make different colors.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of five records for children, and an interview with the provider, the provider did not ensure that all required information was obtained for children's records. Evidence: The record of child 5 did not include written authorization for emergency medical care. The back of the registration form where the parent can complete an authorization was blank.

Plan of Correction: The provider will have the parent complete the written authorization for emergency medical care during today's pick-up time.

Standard #: 22VAC40-111-180-A
Description: Based on a review of three records for caregivers/residents, and an interview with the provider, the provider failed to obtain a current tuberculosis screening and statement for each adult household member every two years from the date of the first screening. Evidence: There was not a TB statement in the file for resident 3. The provider confirmed that a P.P.D. was obtained in the past and could not confirm the last date of a screening. The provider indicated she was not familiar with stored older records. Resident 3 has lived in the home since 2007. In addition, the most recent TB statement for caregiver/resident 1 was completed on January 3, 2018. The most recent TB statement for resident 2 was completed on January 18, 2018.

Plan of Correction: The provider immediately arranged with a family member to assist with obtaining a TB screening and statement for caregiver 3. The provider will arrange for caregiver/resident 1 and 2 to obtain updated TB screenings and statements. The provider will make sure all resident records are maintained and kept up to date as required.

Standard #: 22VAC40-111-350-E
Description: Based on measurement of the hot water temperature at a sink available to children, and an interview with the provider, the provider did not maintain the hot water temperature within a range of 105 degrees F to 120 degrees F. Evidence: The hot was temperature at the downstairs bathroom sink was 127 degrees F. The provider confirmed that the city added a monitoring device to the hot water heater in 2019.

Plan of Correction: The provider immediately adjusted a dial to turn down the water temperature. The provider will check the temperature of the water to make sure it is not too hot.

Standard #: 22VAC40-111-500-C
Description: Based on observation of the outdoor play equipment, and measurement of the use zones of equipment, the provider did not ensure that all equipment was not installed over any hard surface and was at least six feet from obstacles. Evidence: The provider confirmed that a new play structure was installed in the back yard. The two to-fro swings, a hammock swing and long riding swing seat were observed in use and had areas of hard packed dirt underneath. Also, the attached slide exit area had an area of hard packed dirt. Adjacent to the swing set, there was a plastic orange slide and smaller yellow slide that were one to three feet from concrete path stones.

Plan of Correction: The provider will move individual slides away from the concrete path. The provider will either add grass, turf or mulch to the areas under and around the play equipment.

Standard #: 22VAC40-191-40-D-1-a
Description: Based on a review of three records for caregivers/residents, and an interview with the provider, the provider failed to maintain completed copies of sworn statements or affirmations and the finding of the Virginia central registry checks as required for all caregivers. Evidence: The file did not include any sworn statements for caregiver 1 The provider said she was not aware of the location of recent sworn statements. Also, the findings of past searches of the Virginia central registry were not in the file. The provider indicated she did not know where they were kept.

Plan of Correction: The provider will print sworn statements from the D.S.S. website and make sure the statement form is completed and signed. The request form for the Virginia central registry search will be printed, completed and sent to the agency in Richmond. A copy of the sworn statement, Virginia Central Registry request form and the results of the background check will be shared with the licensing inspector. The sworn statement and the Virginia Central Registry finding will be kept in the file.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on a review of three records of caregivers/residents, and an interview with the provider, the provider failed to maintain completed copies of sworn statements or affirmations and the findings of the Virginia central registry checks as required for all residents as required. Evidence: The file did not include any sworn statements and findings of past searches of the Virginia central registry checks for resident 2 and 3. The provider said she was not aware of the location of recent sworn statements and the Virginia central registry findings that may have been stored.

Plan of Correction: The provider will print sworn statements and the request forms for the Virginia central registry search from the D.S.S. website and have resident 2 and 3 complete the forms. Copies of the sworn statements and the request forms for the background check will be shared with the licensing inspector. The sworn statements and the findings of the Virginia central registry searches will be kept in the file.
