Comments:
An unannounced monitoring inspection was conducted today between 9:00 am and 12:15 pm. There were 4 children (11 points) present with 2 caregivers. Two of the children were working on an art project with one caregiver and the other two were on the floor with toys and listening to music.The entire physical plant, emergency plans, attendance logs, evacuation logs,children's records, caregiver's records, household member's records, menu's, bottles, cribs and emergency supplies were inspected. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, the family day home failed to ensure that bottles were labeled with child's full name and dated.

Evidence:
1. There was one green Avent bottle in the refrigerator with no name or date.
2. There was one blue Avent bottle in the refrigerator with no name or date.

Plan of Correction: I will remind parents to put the name and date on the bottles.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the provider failed to maintain up to date records for each child enrolled.

Evidence:
1. Child #3's record does not have documentation of names of person other than the custodial parents who are authorized to pick up the child.
2. Child #3's record does not have documentation of an Information for Parent Form or a Parent Agreement Form..
3. Child #4's record does not have documentation of a medical insurance policy number.

Plan of Correction: I will get these from the parents.

Standard #: 22VAC40-111-240-A
Description: Based on observation and interview with the provider, all equipment in the family day home was not maintained in a safe condition.

Evidence:
Three of the four cribs in the family day home were manufactured 05/2011.

Plan of Correction: Only the 8 month old sleeps in the cribs, the rest sleep on cots. I will take the cribs down.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the family day home family day home failed to ensure that the changing pad was nonabsorbent.

Evidence:
The changing pad has a small hole in it making it non absorbent.

Plan of Correction: I will buy a new pad.
