Each caregiver must obtain 16 hours of training annually.

Comments:
An unannounced inspection was conducted today between 9:50 am and 12:30 pm. There were 11 children (29 points) present with 3 caregivers. The children were observed singing songs, working on the alphabet, hand-washing and eating lunch. Lunch consisted of chicken, rice, pears and milk. The physical plant, children's records, caregiver's records, household member's records, attendance records, menu's, emergency plans, bottles and emergency supplies were inspected. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the Family Day Home failed to maintain an up to date records for each child enrolled.

Evidence:
1. Child #2 and # 4's records do not contain documentation of names of person other than the custodial parents who are authorized to pick up the child.
2. Child #3's record did not contain documentation of an annual information update. The last update was 01/02/2019.
3. Child #2's record did not contain written authorization for emergency medical care should an emergency occur.

Plan of Correction: I will get the information from parents.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver records, the Family Day Home failed to obtain for each caregiver a current Report of Tuberculosis Screening form in accordance with the requirements in 22 VAC 40-111-170 every tow years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence:
Caregiver #3 Tuberculosis Screening has expired.

Plan of Correction: I will have caregiver obtain new screening.
