Comments:
An unannounced monitoring inspection was conducted today from approximately 10:30 am-12:10 pm. There were 12 children (26 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and being fed lunch. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, not all bottles were labeled with the child's full name and date.
Evidence:
Two bottles were observed without a label with the child's name or date.

Plan of Correction: I will remind the parents to label the bottles.

Standard #: 22VAC40-111-120-B
Description: Based on record review, one caregiver record reviewed did not contain all required information.
Evidence:
Adult 4's record did not contain documentation that two or more references were conducted prior to hire.

Plan of Correction: I conducted references, but did not document them. I will document them and add them to her file.

Standard #: 22VAC40-111-60-B
Description: Based on record review, one of five children's files reviewed did not contain all required information.
Evidence:
Child 2's record did not contain the name, address, and phone number for one of the two required emergency contacts.

Plan of Correction: I will ask the parents to provide the missing information.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and provider statement, the provider failed to deny employment to an adult caregiver who does not have a central registry finding within 30 days of employment.
Evidence:
Adult 4, has been employee for a period of over 30 days. A central registry finding is not on file for the caregiver. The provider stated that a central regisrty check has not yet been requested.

Plan of Correction: I did not think this background check was required anymore. I will request one today.
