Technical Assistance:
The provider shall forward each caregiver's remaining required training hour documentation prior to the license expiration on 4/1/20.

Comments:
An unannounced full renewal inspection was conducted today. The provider and two assistant caregivers were caring for 11 children, totaling 31 points. Children's records, children's nutrition, administrative records, background checks, caregiver/household member records, caregiver training, the physical plant, child care programming, equipment, and caregiver interactions were inspected today.

The children were observed playing outside, engaging in creative play, and eating lunch. The children were served chicken nuggets, mashed potatoes, strawberries, and milk for lunch today.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on a review of administrative documents, it was determined that the provider has not reviewed the emergency preparedness plan at least annually.
Evidence:
The provider most recently documented a review of the emergency preparedness plan on 10/5/18.

Plan of Correction: We review it all the time. I will document the annual review.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that four records do not contain all required information.
Evidence:
Child records B, C, D, and E do not include documentation showing that each parent has reviewed their child's contact information within the past year.

Plan of Correction: I will ask each parent to review their child's registration form.

Standard #: 22VAC40-111-280
Description: Based on observations, it was determined that one potentially poisonous material was not stored in a way to keep it inaccessible to the children.
Evidence:
The Inspector observed a large spray bottle of bleach/water solution on the counter directly next to the faucet that is used by the children when washing their hands.

Plan of Correction: The bottle of cleaner was immediately moved.

Standard #: 22VAC40-111-320-B
Description: Based on observations, it was determined that the provider has not covered all electrical outlets with child-resistant covers.
Evidence:
Four electrical outlets on the bottom half of the wall were not covered.

Plan of Correction: I will make sure all outlets stay covered.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on a review of background checks, it was determined that the provider did not obtain two repeat background checks before five years since the dates of the most recent central registry finding.
Evidence:
The Central Registry finding for adult household member A expired on 8/19/19. The Central Registry finding for adult household member B expired on 9/2/19.

Plan of Correction: I will make sure both repeat search forms are sent off tomorrow. I will forward the search request and the findings to my Inspector within 10 days.
