Technical Assistance:
Technical assistance was provided in the areas of diapering, nutrition and fire safety.

Comments:
An unannounced monitoring visit was conducted today between the hours of approximately 10:45 am through 1:30 pm. There were 10 children (26 points) with the provider and two assistants. A sample of 11 children's files and 3 staff/household members files were reviewed. At the arrival to the family day home the children were cleaning up after free play. Children and caregivers were observed during bathroom breaks, hand washing, reading stories, transition to lunch, lunch and transition to nap. Lunch today consisted of pasta with sauce, ground meat, corn and orange slices. Cribs, the physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. There were no medications on site today. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review annually with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct.
Evidence:
1) The most recent update in the record for Child 4 was dated 08/28/2018.
2) The most recent update in the record for Child 9 was dated 02/11/2019.

Plan of Correction: I will ask the parents to update the information.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's files did not contain all required information.
Evidence:
1) The records for Child 3 and 4 did not contain the policy number for the child's medical insurance, a phone number for one parent's place of employment and the name, address and phone number for an additional person to contact in case of an emergency if the parent cannot be reached
2) The record for Child 7 did not contain the policy number for the child's medical insurance.
3) The record for Child 8 did not contain the policy number for the child's medical insurance and the complete address and phone number of an additional person to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: I will ask the parents to complete the missing information.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider did not ensure that the parents were provided in writing with all the required information before the child's first day of attendance.
Evidence:
1) The records for Child 4 and Child 8 did not contain a signed form of the provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: I will ask the parents to sign the forms.
