Comments:
An unannounced monitoring inspection was conducted today. The provider was caring for one child, equaling 1 point. One additional child was expected to arrive after school.
Children's records, the physical plant, administrative records, emergency preparedness items, children's nutrition, and provider:child interactions were inspected today.
The child in care was to be served pasta with ground beef, broccoli, blueberries, and milk for lunch today.

Violations:
Standard #: 22VAC40-111-110-A
Description: Based on a review of children's health records, it was determined that one immunization record has not been fully completed on the current form approved by the Virginia Department of Health.
Evidence:
Child record A does not include the physician's signature to indicate that the child "has been adequately immunized for his/her age."

Plan of Correction: I will ask the parent to have the health form statement signed by the doctor.

Standard #: 22VAC40-111-80-C
Description: Based on a review of children's records, it was determined that the provider did not document her method of verifying one enrolled child's identification.
Evidence:
In child record B there is no documentation to show that the provider has viewed the child's original proof of identification.

Plan of Correction: I viewed the child's original birth certificate, and kept a copy in the file. But I forgot to document that I viewed the original. Moving forward, I will remember to document each ID and I will not keep a copy.
