Technical Assistance:
Technical assistance was discussed with the provider on the following topics:
- taking attendance within 30 minutes of when the children arrive.
- bottles observed in cribs
- limiting the use of the television/electronics to 2 hours/day.

Comments:
An unannounced monitoring inspection was conducted today between 12:00pm and 2:10pm. There were 11 children present with the Provider and 1 assistant for a total of 26 points within the supervision guidelines. The children were observed playing freely inside the family day home, eating lunch outside and playing in the backyard, watching TV and napping. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-700-C
Description: Based on record review and a review of medication, medication was not returned to a parent within 14 days of the medication authorization expiring.
Evidence: The medication authorization for Child #4 and Child #5 expired 11/30/2019; medication was observed on site today.

Plan of Correction: I contacted the parents today, I'll have the parents get updated forms from the doctor.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #1 file did not contain documentation that the provider has verified the child's age and identity.
2. Child #2, Child #4, Child #5 and Child #6 files did not contain documentation of an annual review of the families emergency contact information.

Plan of Correction: I'll ask the parents to update the forms as soon as possible.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were not stored away from food in areas inaccessible to children.
Evidence:
1. There were various cleaning wipes and air fresheners stored on the diaper changing table.
2. There were various cleaning wipes and sanitizers sitting on the table while the children ate lunch.

Plan of Correction: The cleaning materials were moved during the inspection.

Standard #: 22VAC40-111-600-B
Description: Based on observation, infants spend more than 30 consecutive time during waking hours confined in a piece of play equipment and the intervening time period between confinements was not at least one hour.
Evidence: Child #1 was observed standing in a play/activity chair at the start of the inspection (approximately 12:00pm) watching TV. The child remained in the activity chair to eat lunch and was taken out of the chair at 1:30pm when they were given a bottle to drink. Approximately 10 minutes later the child was finished with their bottle and they were returned to the same activity chair to watch TV.

Plan of Correction: Okay, I understand. We will fix it immediately.

Standard #: 22VAC40-111-680-B
Description: Based on observation, children's hands were not washed with liquid soap and warm running water after diaper changes.
Evidence: Child #7 was observed having their diaper changed. The child was laid down to nap after the diaper change; their hands were not washed prior to being laid down.

Plan of Correction: I did it with sanitizer, but I'll wash with water.
