Licensing inspector discussed the following with the provider:
possible closure, additional training, phase 2 training, additional assistance form local family day home providers, consultation with licensing inspector, requirements for all assistants, background check requirements and the license process.
- Comments:

An announced monitoring inspection was conducted today from approximately 2pm-3:15pm. There were 7 children (24 points) in the direct care and supervision of 2 of caregivers. This was a focused inspection initially intended to only discuss previous violations. Upon arrival licensing inspector witnessed the provider walking swiftly from her car to the to the back of the house with her coat and her purse. A new assistant was working directly with the children. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Evidence:
1.Caregiver A stated that Assistant A was there for today to help and did not have the required contents of the record on file.

C. Verification of age,
D. Job title,
E. Date of employment,
F. An emergency contact,
G. Orientation, and

Standard #: 22VAC40-111-50-A Description: Based on record review, the family day home failed to keep a written record of attendance each day.
Evidence:
The family day home did not keep a written record of attendance for the following days: 02/02-07/2020 and 02/10-14/2020.Plan of Correction: Caregiver A won't be back.

Evidence:
1.Caregiver A stated that Assistant A was there for today to help and did not have the required contents of the record on file.
2. The provider did not initiate a central registry search for Assistant A before Assistant A began working directly with children.Plan of Correction: Caregiver A won't be back.
Standard #: 63.2-1720.1-B-2 Description: Based on record review, the provider failed to ensure that each caregiver submit to fingerprinting before working directly with children.
Evidence:
1.Caregiver A stated that Assistant A was there for today to help and did not have the required contents of the record on file.
2. Assistant A did not have complete fingerprinting results before the first day of working directly with children.Plan of Correction: Caregiver A won't be back.
