Comments:
An unannounced renewal inspection was conducted today between the hours of 9:30am through 12:30pm with the provider on site. There were 8 children (23 points) with the provider and assistant. A sample of 7 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, dancing, singing, tv time, diaper changes, bathroom breaks and hand washing. Interactions between the children and staff were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-800-B
Description: Based on review of the emergency plan, the provider did not review the emergency plan at least annually and document in writing each review and update to the emergency plan.
Evidence:
1. The emergency plan was last updated and documented on 2/27/2019 when the inspector arrived for the inspection..

Plan of Correction: It was corrected immediately.

Standard #: 22VAC40-111-70-B
Description: Based on review of five childrent's records, the provider did not obtain the parent's written acknowledgement of the receipt of liability insurance declaration.
Evidence:
1. Child #1 and Child #7 did not have the parent's signature on the liability insurance declaration form.

Plan of Correction: I will get that done today.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas and furnishings of the family day home inside were not maintained in a safe and operable condition.
Evidence:
1. The carpet near the door to go to into the hall towards the bathroom is unraveling and is a tripping hazard. There is also a large missing section of the carpet near the pole in the middle of the classroom approximately 18 inches by 2 inches and is a tripping hazard. The carpet is also raised on the left side of the play area near the tile and is a tripping hazard.

Plan of Correction: I will try and replace the carpet.

Description: Based on observation, potentially poisonous substances such as cleaning agents were stored in an area that was not inaccessible to children.
Evidence:
1. One bottle of Febreze, one container of germicidal disposable wipes, germ hand saitizing wipes and bleach water were on a shelf in the play area to the right of the changing table and was accessible to children.

Plan of Correction: It was moved immediately.

Standard #: 22VAC40-111-680-B
Description: Based on observation and interview, the caregivers did not have an infant wash their hands after diapering.
Evidence:
1. Both caregivers agreed that Child #4 was placed back on the carpet to play without washing the child's hands first after the child's diaper was changed..

Plan of Correction: The child's hands were washed after the reminder and cleaned and sanitized all the toys.

Standard #: 22VAC40-80-120-E-5
Description: Based on observation and interview, the provider failed to post a copy of the notice of the Commissioner's intent to revoke the provider's license in a prominent place at each public entrance of the home to advise consumers of serious violations.
Evidence:
1. Caregiver #1 stated she did not have the Notice of Intent posted. I had it posted, but I took it down to make a copy of it.

Plan of Correction: I will get the paper and put it back up there today.
