Technical Assistance:

Discussed the requirements for household children regarding maintenance of records. Household children are counted in points but not in total capacity.

Comments:

An unannounced Monitoring inspection was conducted this date. A total of 10 children, 1 child is a household member. (20pts) Provider and 1 Assistant on site. When the Inspector arrived, the children were sitting at the round table with staff playing with toys. Some of the older children were dancing and singing in the other programming room. Walk thru of the entire programming space to include the crib room, bathroom, food preparation area and both programming rooms was made. Currently all children are napping on cots, cribs are not in use at this time. The children split into two groups, the younger children were using dolls and a doctor kit using stethoscopes with staff. The older children were matching cards with objects and words. A nice transition was made to tactile activities such as stacking color tubes to create patterns and creative objects. Geometric wood shapes were used to create larger geometric shapes. A total of 5 children's records, both staff and 1 household member record were reviewed. No medication or topical ointments on site at this time. Description:	Based on child record review, two records were incomplete. Evidence: Child #1's record has one of two required emergency contact phone numbers. Child #3's record does not include the following: a signed parent acknowledgement form, proof of identity, a signed liability insurance disclosure and a signed medication policy.
Plan of Correction:	I will obtain all signed information from the parent.
Standard #:	22VAC40-111-80-A
Description:	Based on child record review, verification of the identity of one child was not obtained within 7 business days of the child's first day of attendance. Evidence: Child #3 began care on 12/30/19. Verification of the child's identity has not been obtained.
Plan of Correction:	I will obtain the verification of identity this week, if parent''s are unable to provide the verification, I will be required to call law enforcement.
