Comments:
The licensing inspector conducted an unannounced renewal inspection on Monday, March 2, 2020 between approximately 11:30 am to 1:30 pm. During this inspection, there were five children in care totaling 13 points being supervised by the caregiver. The licensing inspector observed the children in care transitioning from lunch time, engaging in active play with age appropriate equipment, completing bathroom breaks, caregiver diapering and feeding, and transitioning into napping. The licensing inspector inspected all designated child care areas at the family day home. The first aid kit and emergency supplies, posted emergency contacts, emergency drills were reviewed by the licensing inspector. During this inspection, there were four children's records, one caregiver's record, and three household member's records were reviewed by the inspector. The Department has received a complete renewal application and annual fee from the center. Standard #: 22VAC40-111-920
Description: Based on observation and interview, the caregiver did not ensure the menu for the current one week was complete.

Evidence:
1. A review of the menu for March 2-6 was documented with food items for only Monday-Wednesday.
2. Interview with caregiver confirmed the menu for current one-week period was incomplete.

Plan of Correction: Plan of corrections will be provided at a later date.

Standard #: 22VAC40-111-210-A
Description: Based on review of training documentation and interview, the caregiver did not ensure to obtain a minimum of 16 clock hours of training annually.

Evidence: 1. There were 4 hours out of the 16 hours of training reviewed for completion annually. 2. Interview with caregiver confirmed there was no additional documentation of annual training available during the inspection.

Plan of Correction: Plans of correction will be provided at later date.
