Technical Assistance:
Inspector and provider discussed the importance of structured programming and the importance of ensuring all paperwork is completed for accidents.

Comments:
A complaint investigation was initiated on February 5, 2020 with another local agency regarding supervision and injury reports. Interviews were conducted. Documentation was reviewed. There was not a preponderance of evidence to support the allegation regarding supervision. There was a preponderance of evidence to support the allegation regarding injury reports. Therefore the was deemed valid with regard to not completing injury reports. An area of non-compliance is identified in the Violation Notice.

Description: Based on record review and interview, the provider did not record in the child's record an injury or accident sustained by a child while at the family day home.

Evidence: The provider stated they did not provide a injury report for a accident which occurred on 1/31/2020. The provider stated there were incidents in the past that occurred which they did not provide incident reports to parents.
