Comments:
An unannounced renewal inspection was conducted between the hours of 9:05am through 11:00am. There were a total of 8 children present (20 points) ranging between ages 7 months and 5 years old with the Provider and 1 assistant. A complete inspection of the programming space, 5 children's records, 2 staff records, 6 household member files, emergency drills, menus, attendance, and emergency procedures were completed today. Both caregivers on site are current in CPR/First Aid training. The children were observed engaging in free choice play and interacting with caregivers. Positive interactions between caregivers and children were observed during this inspection. The provider advised she does not administer medication and there was no medication on site today. There was an abundant supply of age appropriate materials and toys and the home was clean and organized. Please contact me with any questions or concerns at morgan.bryson1@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-830-C
Description: Based on review of the emergency drill log, documentation was not maintained for all monthly emergency evacuation drills.

Evidence: The provider advised she did complete fire drills in the months of October 2019, November 2019, and December 2019, however, no documentation of these drills was maintained.

Plan of Correction: I will make sure to document every month when I do a fire drill.

Standard #: 22VAC40-111-180-A
Description: Based on record review, 3 of 3 adult household member records reviewed did not contain documentation of a tuberculosis screening or test completed within the last two years.

Evidence: Household Member #4's last tuberculosis test was completed 4/6/17, Household Member #5's last tuberculosis test was completed 5/11/17, and Household Member #6's last tuberculosis test was completed 2/9/18.

Plan of Correction: I'm going to have them complete the tuberculosis tests.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on review of 6 household member's records, it was determined not all household members had the results of a central registry search on file by the age of 14 years old.

Evidence: Household Member #1 (Age 14), Household Member #2 (Age 15), and Household Member #3 (Age 16) did not have the results of a central registry search on file.

Plan of Correction: I already sent the central registry search request and I'm waiting for the results.
