Comments:
An unannounced renewal inspection was completed today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 2:15-3:50pm. There were 9 children, totaling 23 points, under the supervision of 3 caregivers. Today the children participated in circle time, played with Play-doh and puzzles, listened to music, rested during naptime and enjoyed free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served macaroni and cheese, mixed vegetables, cantaloupe, apples and bananas and milk for lunch. The Provider is certified in MAT (medication administration training). Standard #: 22VAC40-111-940-A
Description: Based on observations, the refrigerator did not have a temperature maintained at or below 40 degrees.
Evidence
1. The refrigerator that the Provider uses to store food for the children in care had a thermometer which read 44 degrees.

Plan of Correction: The thermostat will be lowered.

Standard #: 22VAC40-111-60-A
Description: Based on file review, 1 out of 5 children's records has not been updated annually.
Evidence
1. Child #3's record was last updated on11/13/18.

Plan of Correction: This will be updated.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 1 out of 5 children's records do not have all required information on file.
Evidence
1. Child #4 does not have phone numbers for the 2 emergency contacts in case the parents cannot be reached or the child's physician's address and phone number.

Plan of Correction: I will get this from the parent.

Standard #: 22VAC40-111-210-A
Description: Based on interview, the caregivers did not obtain a minimum of 16 hours of training annually.
Evidence
1. The Provider stated that all 3 caregivers have not completed 16 hours of training in 2019.

Plan of Correction: We will complete this requirement next year.

Standard #: 22VAC40-111-520-E
Description: Based on observations, the Provider failed to ensure that bumper pads were not used on the cribs.
Evidence
1. There were 2 cribs, occupied by children, which had bumped pads tied to the top panels of them.

Plan of Correction: These will be removed
