Technical Assistance:
LI had a discussion with the provider in reference to the diaper changing surface

Comments:
An unannounced monitoring/renewal inspection was conducted today between 2:20pm and 3:50pm. There were 2 children present with the Provider for a total of 7 points within the supervision guidelines. The children were observed napping during today's inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider has not obtained a physical exam prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #2 (date of enrollment: 9/3/19) file does not contain documentation of a physical exam.

Plan of Correction: I will text the parents right now to let them know what I need.
