Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and parking on the street during school hours. Discussion was also held on the renewal application, and using safe practices, washing hands in order to combat Covid 19.

The following inspection took place between the hours of 9:30 am through 12 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Sterling, Virginia. At the start of this unannounced renewal inspection there were a total of 5 children observed (8 points) with 1 caregiver. A total of 5 children?s files, 1 staff file, and 3 household member?s files were reviewed. The children were engaged in the following: learning, games, toy play, music, song, a St. Patrick day activity, movie time, bathroom breaks, hand washing, snack, and other organized activities. Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 5 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (03/17/2020), the written authorization for emergency medical care (should an emergency occur and the parent cannot be located immediately) was not available for review for Child #5.
2. A written objection to the provision of medical treatment on religious or other grounds by the parents was not present.
3. The start date for Child #5 was listed as - 07/11/2017.

Plan of Correction: This documentation shall be obtained and placed on file.

Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 3 household member files indicated that the Provider did not obtain for adult household member a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most recent TB documentation.

Evidence -
1. On the date of inspection (03/17/2020), the most recent TB documentation on file for Household Member #1 was dated - 12/07/2017. This documentation expired in December 2019.
2. The Provider confirmed this review on the date of inspection.

Plan of Correction: This household member shall obtain current TB documentation and place it on file at the Family Day Home.

Standard #: 22VAC40-111-210-A
Description: Based on review and interview, in addition to satisfactory completion of first aid training and CPR training, the caregiver did not obtain a minimum of sixteen (16) clock hours of training annually in areas relevant to their job responsibilities.

Evidence -
1. On the date of inspection (03/17/2020), there were no training certificates available for review for the past annual training year.
2. The Provider stated (during an interview conducted on the date of inspection) that Staff #1 was unable to complete the required training hours for this annual training year.

Plan of Correction: A total of 16 hours shall be completed for the current training year.
