Technical Assistance:
Discussed with the provider the distance required between the children's outdoor play area and the pool area. The children will only use the front porch and the front yard when playing outdoors.

Comments:
An unannounced monitoring inspection was conducted today from 10:35am-11:50pm. There were 2 children (4 points) directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and reading books. All daycare areas were inspected along with a walk through of the smaller dwelling located to the rear of the home. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's record did not contain all required information.

Evidence:
Child#1's record is missing the mother's work phone number. Child#2's record is missing the mother's work phone number and the child's medical insurance policy number.

Plan of Correction: We will request the missing information.

Standard #: 22VAC40-111-70-A
Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance.

Evidence:
Child#2's record is missing the family day home's policies for the administration of medication, the Information for Parents form, provisions of the family day home's emergency preparedness and response plan and the Liability Insurance Declaration form.

Plan of Correction: I will give the documents to the parents tonight.
